A signature is required for each student on the following forms:
All required forms are also available at in-person lessons and can be provided by either your teacher or a member of our front office staff.
✅ Please complete all paperwork prior to your first lesson.
You may either:

Fill out the forms electronically and email them back to our office
OR -
Complete them in person at your first lesson
